This page contains the different forms needed for repeater coordination within Texas.
Click the link to view the forms, or right click to download and print them. They are encoded with Adobe Acrobat, and you will need the latest version of the free viewer to view and print them.
All forms must be printed, signed, and returned to the appropriate ZFC for processing.
To find out which zone you are in, check the Zone Map.
To coordinate a new repeater, you will need the Construction Permit Application form.
If for some reason you need to extend (one time only) your construction permit, you will need the Second Construction Permit Application form.
If you wish to modify the technical parameters of your currently coordinated repeater, you will need the Repeater Technical Change form.
If you wish to transfer the coordination from an individual to a club, you need the Individual to Club Transfer form.
If you are a club, and you need to change the designated trustee, you need the Club Coordination Trustee Change form.
If you are a trustee and need to report a change in your machine's status, you need the Repeater Status form.
If you are a member wishing to report the on-air status of a repeater, you need the Repeater Status form.
If you are ready to retire and let the machine go, you need the Coordination Relinquish form.
